Cross-border healthcare within the EU (EU-S2): Your guide to using your right to receive treatment outside Germany
What is the EU-S2 form?
EU-S2 (formerly E112) is an official form that allows you to receive planned medical treatment in another EU member state, paid by Germany’s statutory health insurance (GKV)—only with prior approval.
When can EU-S2 be used?
-
when the treatment is not available in Germany within a reasonable time
-
or when an EU center offers more specialized, better, or faster care
-
it applies to organized/planned care only (not emergencies)
Examples of covered treatments
| Treatment | Covered under EU-S2? |
|---|---|
| Cancer surgery in France | Yes |
| Organ transplant in Belgium | Yes |
| Simple physiotherapy in Spain | Often no |
| Emergency care while on holiday | No (covered via EHIC) |
Key requirements
-
a clear diagnosis from your doctor in Germany
-
a defined treatment plan from the EU hospital
-
approval from your German health insurer (Krankenkasse) before treatment starts
-
use of the official EU-S2 form
How to apply
-
See your German doctor → obtain a medical report
-
Find an EU hospital that provides the treatment
-
Request a written cost plan (Kostenplan)
-
Submit all documents to your GKV insurer
-
If approved, you receive a signed EU-S2 form
Who pays the costs?
-
the German insurer pays the foreign provider directly
-
you may still have to cover additional costs (e.g., single room, travel, translation)
Important notes
-
the form does not apply to non-EU countries, and it is not used for the UK after Brexit
-
the insurer may refuse if there is a suitable alternative within Germany
-
traveling without prior approval usually means you pay yourself
