Cosmetic Surgery in Germany – Why Insurance Rarely Covers It

Statutory health insurance (GKV) only covers medically necessary procedures. Purely cosmetic surgeries are considered non-essential.

Coverage Criteria Only if there is a clear medical reason or proven psychological suffering.

Examples not covered

  • Breast augmentation → aesthetic only

  • Liposuction → non-therapeutic

  • Facelift/eyelid lift → cosmetic only

  • Rhinoplasty → covered only if breathing impaired

  • Wrinkle removal/fillers → optional

When covered

  • Correction of congenital defects (e.g., cleft lip)

  • Post-accident or burn reconstruction

  • Removal of excess skin after bariatric surgery causing infections

  • Breast reduction for chronic back pain

  • Rhinoplasty for severe nasal obstruction ➡️ Requires medical reports and proof.

Psychological reasons Possible with psychiatric report, strict review required.

Costs if self-paid

  • Rhinoplasty: €2,500–5,000

  • Breast augmentation: €4,000–7,000

  • Abdominoplasty: €3,000–6,000

  • Liposuction: €2,000–5,000

Private Insurance (PKV) May cover more, depends on policy, usually needs medical/psychological documentation and preapproval.
