Embassy or employer (HR) letters to guarantee return after medical treatment
When are they required – and what should they include?
When applying for a medical treatment visa for Germany, it is not enough to prove the medical necessity of the treatment. German embassies and consulates also usually require strong guarantees that the patient and any accompanying person will return to their home country once treatment is completed.
One of the most important of these guarantees are official letters from the employer (HR letter) or letters from the applicant’s own national embassy, confirming professional, financial or social ties to the home country.
In line with the requirements of § 14 AufenthV, German missions abroad assess whether there are indications that the visa application might be used as a pretext for irregular immigration.
An employer or embassy letter helps to:
Prove strong ties to the country of origin (job, family, obligations)
Reassure the authorities that the applicant will leave Germany after treatment

the financial situation is not very strong, or
an accompanying person is travelling who is not a first-degree relative

Note: Some embassies issue such letters only in specific cases or upon explicit request from the German hospital.
Do not submit generic or outdated letters – they should be recently dated and case-specific.
All names and details must exactly match those in the passport and visa application.
Have the letters written or translated into German or English, and legalised or certified if required.
Ideally, submit these letters together with the visa application documents, not separately at a later stage.
Employer and embassy letters are not minor documents. They are a key pillar in building trust between the applicant and the German consulate.
